What We Do
Our projects at Jefferson Family Health Fund are making a real difference in the lives of individuals, families and the community. We focus on educating and supporting people, by providing access to resources that are essential for personal growth, better health outcomes as well as creating community connection. Below are some of our projects.

We provided our Diabetic Group with tools to help them achieve better results with diabetes control.
Generations brings experienced retired people together with children and young parents to build relationships and foster mentorship.
We partnered with FoodLink to bring fresh produce to the neighborhood where going to the grocery store is difficult or fresh produce would have been too expensive.
We have partnered with local schools, churches and parents to provide tutoring and mentoring programs for at risk youth.
Working with youth at the community garden so they learn and maintain an interest in gardening and healthy eating.

Reached out to the community to achieve better health outcomes.
With a grant from the
Genesee Valley Medical Foundation, we began our first 11 backyard gardens in 2014. We are pleased to say we've been able to continue those gardens and due to their popularity, have had up to 55 gardens each year through 2023.

We have sponsored community health fairs to educate and provide a strong health foundation for the community.
We have sponsored the Magnolia Children's Garden which is a community garden that has allowed up to 17 families to grow fresh produce over the last 8 years.
In 2018 we were awarded the
Flower City
"Seeds of Change Distinction" Award.
Community outreach to make certain good health begins at the earliest stages of life.
We partnered with the Southwest Area Neighborhood Association (SWAN)
to reach youth with the purpose of guiding them toward possible career paths.
